Your part in our Blue Care team

BlueCare has embarked on an exciting transformation to strengthen the provision of quality care in the community setting. Being part of this newly created centralised function of Statewide Dementia Care team, you will be working with a team of passionate, like-minded professionals. With a dedicated focus on providing high level care based on assessed client needs that promotes quality of life and enhances the ability of individuals to maintain their independence and remain within their home in the community.

Your part in our Statewide Specialised Team

Reporting to the Dementia Care Manager, Statewide Specialised Service, you will support BlueCare's neighbourhoods in in providing, support, guidance and strategies to personal carers, nursing and allied health staff in the delivery of high quality in home and dementia care.

The Dementia Care Consultant is responsible for ensuring the community service delivers quality Dementia care, based on assessed clients' needs that enhances and promotes quality of life, and is consistent with current best practice.

With our clients' wellbeing always in mind, you will deliver direct in-home or telehealth support to people living with Dementia and their families by providing:
The Dementia Care Consultant will deliver direct in-home or telehealth support to people living with Dementia and their families by providing:
- Information and tools specific to individual circumstances.- Assistance to access BlueCare support services and programs to maintain health and well-being.- Carer Support.- Techniques to cope with individual behavioural concerns.- Prescribe meaningful activities and equipment from a comfort interventions library, to engage people living with Dementia.- Care planning and assessment as per review schedule and change in clients' needs.- Linkages and referrals external stakeholders and Government departments in the delivery of specialised dementia care where required.

You'll be an advocate for person-centred care, leading by example through exceptional communication and customer service. Coupled with high level listening and problem solving skills, you will have the ability to develop an understanding of the client's needs and providing quality advise in ensuring our client's ability to self-manage and live more independently. You'll present with:
- Degree level qualifications in a Health, Community Service or other relevant discipline, or extensive relevant work experience.- Certifications or qualifications in dementia care.- Extensive specialist knowledge and experience in Dementia assessment, behaviour management planning, care planning, implementation and evaluation and Dementia mentoring and supervision, allied health or human services with demonstrated skills reflective of significant functional experience where scope of role includes clinical or care staff support.- Expert knowledge of services, tools, techniques and resources to assist clients and staff.- Proven leadership capability within a multidisciplinary team.- Demonstrated comprehensive understanding and knowledge of client
