Our VisionThis role empowers people to live more independently and maintain their quality of life through expert support and high-quality advice.
The successful candidate will have extensive experience in dementia care, providing direct in-home or telehealth support to people living with dementia and their families.
Key responsibilities include delivering support by providing information and tools specific to individual circumstances, assisting clients in accessing support services and programs, and providing carer support and techniques to cope with behavioural concerns.
Prescribing meaningful activities and equipment from a comfort interventions library is also an essential aspect of this role.
Care planning and assessment will be conducted as per review schedule and change in clients' needs.
Linking with external stakeholders and Government departments may be required for the delivery of specialised dementia care.
We are looking for someone who can advocate for person-centred care, leading by example through exceptional communication and customer service.
A strong understanding of client needs and provision of quality advice to ensure self-management and independence are key requirements for this position.
Presenting degree-level qualifications in a Health, Community Service, or other relevant discipline, or extensive relevant work experience is necessary.
Certifications or qualifications in dementia care are highly desirable.
This role requires extensive specialist knowledge and experience in dementia assessment, behaviour management planning, care planning, implementation, and evaluation, as well as dementia mentoring and supervision.
