We enable people with disability who want to live independently by providing flexible options for living arrangements and tailoring supports to match the participants and family's needs.

Within this role, your purpose is to provide support to our NDIS Participants in their own / SIL Homes or in community and coordinate within the support team to ensure smooth delivery of the services.

This position will ensure the completion of applicable administration tasks whilst ensuring the quality of NDIS disability services for all participants. This will enable our participant's' independent living in alignment with individual plans and the service offerings of the home portfolio (operating 24 hours per day, 7 days per week by our support workers). The regular timings for this role will be Monday to Friday 9:00 am-5:00 pm in our Balcatta Office.

**Responsibilities**:

- On-boarding new clients and ensuring that all the required tasks are fulfilled.
- Creating the Service Agreements and updating these when required.
- Administration, supervision and support hours.
- Liaise with support coordinators, families, clients, Pharmacy, GP, OT, Physio and other relevant team members to ensure the smooth service provisions.
- Creating the full client profile in CRM and correctly entering the funding into the system.
- Creating the service booking on My Place portal for NDIA managed clients.
- Supervise site operational activities by doing home visits, including participants service delivery, site safety and safeguarding, site inventory and issue management.
- Support the financial performance of the home service site through effective management of operating expenditure in line with budget.
- Provide support to clients aligned to their personal plans, goals and any other requirements, including but not limited to all aspects of personal hygiene and living skills to ensure a safe and comfortable living environment is provided.
- Supervision and training including regular monitoring of a small team on a rotating roster to maintain effective site operations and resolve issues.
- Ensure team member awareness of individual client's requirements.
- Deliver and encourage others in the accurate documentation of all activities to ensure an accurate audit trail and maintenance of progress notes.
- Medication Management Monitoring.
- Behavior Support supervision and monitoring.
- Ensure all the client's assessments are up to date and the organisation is compliant in-service delivery as per NDIS quality and safeguard commission.
- Monitoring, Reporting and recording of the internal, reportable incidents and Restrictive practices in the Organisation.
- Report writing for NDIS plan reviews and assisting with the review process.
- Ensure continuous smooth service delivery for the clients throughout and assist with problem solving and completing all the requested tasks from the management.
- Ensuring that the compliance are up to date as per NDIS Standards.
- Assisting with complaint handling to reach Participants satisfaction level and improve quality of services.

**Eligibility**:

- Certificate III in Disability and/or Certificate III in Individual Support or any relevant qualification to the related sector is desirable.
- Demonstrated ability in assisting vulnerable people with relevant experience to identify goals and in assisting them to make the necessary choices and decisions to achieve these goals.
- Demonstrated understanding of disability services provision and NDIS Quality & Safeguarding Framework or the ability to develop.
- Ability to supervise the site operations and deliver services aligned with organisational policies and procedures.
- Excellent communication and problem-solving skills.
- A compassionate and client-centered approach.
- Ability to manage conflict whilst remaining calm and alert.
- Flexible and is able to work under pressure with competing demands.
