**About the role**:
The Home Care Coordinator:

- Ensures that care needs are being met through high-quality service delivery
- Supports clients in developing or reviewing their Client Goal/Care Plan
- Coordinates and monitors the delivery of services, maintaining appropriate communication with carers and family members
- Mentors home care workers in the community for their benefit, and that of their clients.

**This is a permanent, full-time position.**

- Can make sound judgements and develop high-level Client Goal plans based on your knowledge of Home and Community Care
- Enjoy working with a team to deliver services
