We are seeking Disability Support Workers with Qualifications in Cert 4 Disability, other equivalent qualification or more than 1 year experience in the field to provide direct assistance support and care for people living with disability impairment or limitation. These roles often involve personal care, in home care, community access support, and assistance with everyday tasks and activities. Overnight (sleep over) and daytime support may be required.

**About you**: You are motivated, reliable, flexible and work well in a team. You enjoy helping others and have a Certificate 4 in Disability (equivalent or higher). Applicants must have a current police check, driver's license and level 2 first aid

- A Certificate IV in Disability or equivalent

**About Us**: Aspire Support provides personal in home and community support services for people living with various impairments or limitations in Melbourne. Aspire Support is a boutique person centred provider that is guided by the needs and choices of the participant being supported.

Aspire Support is a NDIS TAC and Workcover provider. We are receiving referrals for attendant care/ direct support services and are creating a group of professional caring support workers. Positions will be created specific to each participants individual needs and location.

**Position Description**

**JOB Title: Support Worker / Carer**

**JOB TYPE: Full-time; Part-time; Casual (as required)**

**LOCATION: Client's home or in the community as required, Melbourne - South east, Mornington Peninsula, Eastern suburbs, Bayside**

**SUPERVISOR/MANAGER: Managing Director**

**Role activities**:

- Provide participant centred support and assistance as specified in participants care plan
- Responsible to ensure the safety and wellbeing of participants at all times
- Reporting to team leader
- Perform duties as directed by supervisor

**MAIN DUTIES/RESPONSIBILITIES**
- To plan and regularly review with participants goals and plans which contribute to a life of meaning for that individual and which extends their community integration.
- To support the development and implementation of plans supporting a person-centred approach and contribute to the review of the customer's progress.
- To develop appropriate, supportive working relationships with customers and their families/carers.
- To provide support which reflects the Disability Services Standards, legislative requirements, funding objectives, plans and goals and to report any potential breaches to HWNS management.
- To actively promote HWNS Vision, Mission and Values in everyday work and in the community.
- To follow, contribute and work in a culture committed to continuous improvement.

**Qualifications**
- Cert IV or higher in disability, mental health, community services or relevant ertiary qualifications

**Competencies and experience**

ADDITIONAL QUALIFICATIONS AND EXPERIENCE
- Current National Police Check, Working With Children Check
- Level 2 First aid certificate
- NDIS Worker Orientation Module - Quality, Safety and You (Online Training)
- Covid-19 Infection Control Training (Online Training module)
- Knowledge of NDIS, Victorian health systems and local services
- Ability to work with people from diverse backgrounds
- Understanding of evidence-based practices
- IT competent
- Current driver's licence & access to a comprehensively insured vehicle
