About the role
The purpose of the Advanced Social Worker is to deliver an advanced level of quality social work services as a member of a hospital Social Work team within a multidisciplinary environment and lead innovative clinical service development to meet the West Moreton Hospital and Health Service strategic plan and National Health Reform Agenda, also provide active leadership in clinical services, professional support and membership of relevant committees or networks for Social Work Services at a HHS level and a state-wide level as appropriate.
About you
**Clinical Practice and Knowledge**:

- Demonstrated advanced level of experience in delivering a contemporary social work service in triaging, psychosocial assessments, care planning, case coordination and the development of a responsive and high quality service in supporting patients, families and their carers across the HHS.
- Demonstrated commitment and leadership for active and ongoing professional development in meeting the CPD hours set by the AASW.

**Patient Focus**:

- Demonstrated ability to effectively prioritise and manage own time to meet and exceed customer expectations, with an enthusiastic, calm and flexible approach.

**Communication and Team Focus**:

- Demonstrated high quality service provision of practice within the medico legal requirements and AASW standards.
- Demonstrated high level of communication, negotiation, conflict resolution and consultative skills to independently and effective problem solve and contribute positively in all interactions in the workplace consistently, with an ability to work effectively within a multidisciplinary health care team and contribute to the professional development of self and others in a dynamic environment.

**Leadership and Management**:

- Provide an advanced level of clinical leadership in the supervision, performance appraisal and development and performance management of staff to support effective and highly accountable social work practice.
- Demonstrated flexibility and willingness to adapt and lead proficient change management agendas set by the HHS or Discipline Lead and evidence of the ability to evaluate and implement practice changes.

**Quality Focus**:

- Demonstrated commitment to self-development and improving skills to enhance care delivery, and contribute to quality improvement activities, including providing advice regarding service delivery matters affecting cancer patients, and their families and carers.

**Mandatory Requirements**:

- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
- Vaccinated against COVID-19 as outlined in the Health Employment Directive NO.12/21and the Queensland Health Human Resources Policy B70.
