**Purpose of Role:
As the Workplace Adjustments Delivery Lead, you will provide case management and support team members with disabilities and/or neurodivergence across Australia and New Zealand. You will play a key role in ensuring team members receive the workplace adjustments they need to succeed and thrive at Woolworths. You will partner with the Injury Care team, internal stakeholders and external providers.

Key Responsibilities:

- Responsible for effective case management and coordinate assessments when required
- Effective case management and coordination of Workplace Adjustments
- Provide a variety of assessments depending on your area of clinical expertise. This could include functional capacity evaluations, worksite and ergonomic assessments.
- Assist managers in supporting team members with disability
- Manage the intersection between a workplace adjustment and possible JobAccess funding, together with knowledge of relevant legislative frameworks
- Manage and coordinate moving approx. 200 team members from supported wages to open employment conditionsProvide disability-specific secondary consultation to support broader teams to provide more accessible, safe, and inclusive care to people with a disability.

**About You:
- Experience providing case management and/or support services to people with disability, preferably in a workplace context.
- Knowledge of challenges and supports available to individuals living with disabilities, such as JobAccess and NDIS referral pathways and supportAn understanding of disability employment and the ability to provide support to people with disability, neurodivergence, long term medical/chronic conditions and/ or mental health conditions in a manner which is empathetic, sensitive and supports their agency.

**Desirable:
- Demonstrated experience in the disability, advocacy and health sphereDemonstrated experience in project management and project delivery including evaluation processes and the ability to maintain accurate records for reporting and future planning purposes
