Aligned with CALHN's strategic vision for delivering world-class healthcare, Hampstead Centre is launching a new program, "CO-ACT" (Care of the Older Person and Community Transition), focused on providing exceptional care for older adults transitioning from acute to ongoing care. **About the Role**

We are seeking a passionate **Lifestyle Activity Coordinator** to join our team, focusing on enhancing the health, wellbeing, and quality of life of patients within the CO-ACT program. In this role, you will design and coordinate group-based activities that encourage patient participation and help maintain physical, social, and emotional wellbeing. Working closely with Allied Health professionals, you'll develop holistic programs that cater to the unique needs of each patient and align with our site's care goals.

As a key part of the CO-ACT team, you'll also support Allied Health services by assisting with patient care activities under the supervision of a qualified professional. **About You**

We are looking for an enthusiastic and compassionate Lifestyle Activity Coordinator with the following key attributes to join our team:

- **Strong Interpersonal and Communication Skills**: High-level verbal and written skills, with the ability to connect effectively with diverse individuals and write clear, concise reports.
- **Adaptability and Problem-Solving Abilities**: Demonstrated flexibility in responding to changing workplace needs and an aptitude for resolving challenges using relevant policies and guidelines.
- **Team Player with Independence**: Proven capability to work both collaboratively and autonomously, delivering quality care in a supportive healthcare environment.
- **Healthcare Experience and Knowledge**: Prior experience in aged care or a healthcare setting, with an understanding of confidentiality, ethical practice, and the specific care needs of older adults.
