The Integration Officer Multidisciplinary Teams is responsible for guiding and facilitating effective engagement with the allied health sector in western NSW PHN region to strengthen multidisciplinary team-based care in primary care. This role will provide leadership on coordination, advocacy, education and establishment of allied health partnerships with General Practices and ACCHOs to influence and support implementation of evidence-based care. The Integration Officer Multidisciplinary Teams will be guided by the 10-Year Primary Health Care Plan and the National PHN Allied Health in Primary Care Engagement Framework to strengthen our engagement with the allied health sector within the WNSWPHN region.
You will have the ability to build effective relationships with clinicians, stakeholders and communities to understand key challenges and opportunities to strengthen multidisciplinary team-based care in primary care settings, utilizing critical enablers such as digital health, continuous quality improvement activities, education and commissioning.
Selection Criteria
Essential
· Experience working in and/or knowledge of primary care, such as General Practice, Aboriginal health services, allied health services, community pharmacy or Community Health services.
· Excellent interpersonal skills, with well-developed written communication skills including writing project reports and management briefings and development of project materials.
· Experience in project management, including creating and managing project timelines.
· Demonstrated understanding of Aboriginal culture including a commitment to cultural awareness and safety.
· Demonstrated high-level interpersonal and communication skills and high-level competence in using information communication technology.
· Demonstrated organisation and time management skills, with proven ability to deliver high quality outcomes in agreed timeframes.
· Capacity to travel within western NSW; hold a current driver's licence.
Desirable
· Experience working in the health, NGO or community services sectors
· Qualifications in project management and/or experience in project management
· Experience in working with Aboriginal people, organisations and communities in view of planning and implementing services and achieving outcomes for Aboriginal people.
· Experience working in the health, NGO or community services sectors
· Demonstrated partnership development experience
