Healthscope's Victorian Rehabilitation Hospitals encompass Dorset Rehabilitation Centre, located in Pascoe Vale, and The Victorian Rehabilitation Centre located in Glen Waverley.

The Rehabilitation Hospitals offer an extensive range of inpatient and outpatient services. The specialties include multi trauma, orthopaedics, stroke, cardiac, restorative, respiratory and pain management.

Job Description

Healthscope is seeking passionate and experienced Registered Nurse or Allied Health Professional to join us as a Rehab Assessor across our two Victorian rehabilitation hospitals to cover a maternity leave of absence.

We welcome applications with rehabilitation experience who are committed to delivering exceptional patient care.

Working within a multidisciplinary team environment, you will join a patient-centred team caring for patients who are recovering from surgery or illness or needing support to maximise their independence.

Rehabilitation Assessor role will be to facilitate coordination and continuity in the provision of patient assessment, referral and transfer from acute hospitals to one of our two rehabilitation centres.

Key Responsibilities:

  • Reviewing referrals from external stakeholders and ensuring timely and professional assessment for of potential patients
  • Making clinical decisions determine the best rehabilitation pathway for each patient, and assist in identifying goals to achieve during admission
  • Developing collaborative relationships with the external community stakeholders and within our multiciliary team

Essential Criteria

  • Minimum of 3 years' experience in acute or rehabilitation nursing.
  • Demonstrated knowledge of current rehabilitation practices and evidence-based care.
  • Recognised degree in Nursing or Allied Health fields
  • Current registration with AHPRA (Australian Health Practitioner Regulation Agency).
  • Demonstrated clinical assessment skills
  • Exceptional communication and customer service
  • Strong communication skills, both verbal and written.
  • Ability to work autonomously or within a small team.
  • Capability to work efficiently under pressure.
  • Excellent interpersonal skills, with the ability to build rapport with patients and healthcare professionals.
  • Strong organizational and time management skills.
  • Ability to prioritize and manage a diverse caseload effectively.
  • Valid driver's license.

Desirable Criteria

  • Experience in Cardiac or Medical Nursing
  • Knowledge of Health Fund contracts
  • Previous experience in the Private Health sector
  • Demonstrated knowledge of current rehabilitation practices and evidence-based care.
