The Role
The Credentialled Diabetes Educator will work collaboratively within a multidisciplinary team to provide clinical support to clients referred to Black Swan Health's Health and Therapy Services. They will provide best practice education and advice in diabetes self-management to a range of clients in both individual and group settings.
The successful candidate will
- Provide high quality, evidence based, diabetes self-management education, advice, and support to clients on an individual basis and in a group setting.
- Assist and educate patients in the use of relevant blood glucose monitoring.
- Provide insulin initiation and titration support as per instructions from the client's GP/Specialist.
- Deliver training modules/courses in diabetes management to the NDIS, aged care work force and health professionals in-house and at external locations e.g. RACFs, care homes and other community-based cations as required.
- Liaise with other diabetes service providers and community services to ensure an integrated diabetes service.
- Have experience in planning, evaluating, and delivering evidence-based training in diabetes management to health professionals.
- Possess the relevant tertiary health professional qualification with Credentialled Diabetes Educator status and current registration with ADEA.
- Have a valid Driver's license and own vehicle
