About the Team

Communities at Work Disability Services provides programs that support, educate and guide children, young people and adults living with a disability to develop and enhance their social skills, build peer networks and work towards individual NDIS goals.

About the Role (Full Time Fixed Term - 12 months)

The Purpose, Mission and Values of Communities at Work supports clients to exercise informed choice and control through a person-centred approach. The role requires an active and collaborative approach to risk assessment, supports development, planning and outcomes evaluation, for all clients with varying complexities of need.

The Senior Program Facilitator is responsible for the day to day running of the program, mentoring team members, working with clients and their stakeholders, and delivery of face-to-face service supports. You will contribute to the planning and be responsible for the implementation of program allocations, schedules, and activities.

You will be required to connect with clients and other stakeholders on an individual/small group basis, to understand their needs, create an environment that enhances learning and significantly improves the lives of vulnerable people in our community.

Duties and Responsibilities

  • Work directly with persons with disabilities using a best practice approach that is working toward outcomes that are consistent with individual goals and aspirations
  • Ensure team members and yourself work in accordance with the requirements of the NDIS Framework, the Quality and Safeguards Commission, The Children's Education and Care Standards
  • Plan and deliver workshops and activities within the NDIS framework to provide opportunities for Life Education in After School Care, School Holiday programs and day programs for Adults
  • Facilitate agreed group activities/workshops that could include, but are not to be limited to, Home skills, community skills, social skills, work skills, sensory and recreation skills.
  • Conduct an evaluation of activities to ensure they are engaging, challenging and caters to the individual needs of clients
  • Provide structured and unstructured opportunities to have new experiences and learn new skills, whilst respecting their rights and providing positive role modelling
  • Prioritise the physical and psychological safety of participants at all times by planning and delivering activities using the risk management framework
  • Be responsible for the monitoring and record keeping for clients and their relevant information on their participation in the program/s. This includes recording changes in client needs and improvements in individual participant on each attendance and by contributing to the development of individual opportunities plans and plan reviews
  • Work collaboratively with other staff members to maintain staff ratios and skills mix to provide a safe physical and psychological environment
  • Treat all participants, their families and other stakeholders with respect and be inclusive of different cultures, religions and beliefs.
  • Actively engage in and be responsible for the completion of administrative requirements in a timely manner; including incident and accident forms, risk assessment, program reviews and other activities.
  • Engage actively with the Communities at Work Performance Development Framework (PDF) and participate in identified professional development activities
  • Contribute to the maintenance of the overall Communities at Work environment and the Disability Program equipment and premises.
  • Credibly represent Communities at Work and the Disability Program in a professional manner when in public, at relevant forums or when attending training.

Skills and Experience

Essential

Maintain current "Working With Vulnerable People" registration

Maintain a Current First Aid Certificate

Maintain a current Food Safety Supervisor Certificate

An appropriate Certificate IV or above qualification in disability services

Proven experience of working with persons with disabilities

Ability to work directly with persons with disabilities using a best practice approach that is consistent with the National Standards for Disability, NDIS Quality Safeguards Commission – NDIS Practice Standards and Code of Conduct.

Knowledge of or the capacity to rapidly acquire awareness of a person-centred approach and the capacity to delivery supports consistent with this approach.

Commitment to continuing professional and personal development to ensure the provision of best practice services.

  • Current drivers licence
  • Proven experience in leading a small team
