We're now looking for a Service Delivery Coordinator (Intake and Review) to join our Care Services team in a part-time, fixed-term role (12 months, approx. 32.5 hours per week). In this role, you'll be the warm and reassuring first point of contact - helping people take their first steps with us and ensuring they feel guided every step of the way.

What You'll Be Doing

In this role, you'll often be the first point of contact for our clients and their families. You'll provide clear information, timely responses, and supportive guidance, helping people feel at ease from the very beginning. Whether it's coordinating referrals, managing care plan reviews, or simply offering reassurance over the phone, you'll ensure every interaction is handled with both professionalism and care.

Key responsibilities include:

  • Following up on referrals and care plan reviews within required timeframes
  • Checking in on client progress, incidents, and safety assessments to support wellbeing and compliance
  • Providing accurate advice on care plans, service options, and associated costs so families feel confident in their choices
  • Handling calls and call-backs with professionalism, empathy, and understanding
  • Responding to emails within a 24–48 hour timeframe
  • Supporting smooth, respectful client onboarding experiences
  • Working closely with the Intake & Review team to ensure every client feels supported from their very first contact

What You'll BringWe're looking for someone with both heart and skill - a person who is organised, caring, and ready to make a real difference for our clients. You'll have:

  • Certificate III in Aged Care (or equivalent qualification)
  • Statutory Declaration
  • Current National Police Check
  • Current First Aid and CPR certification
  • Driver's Licence
  • Experience in administration, intake, or scheduling within a busy environment
  • A genuine passion for supporting people's health and wellbeing
  • Experience in community services (preferred)
  • Familiarity with the My Aged Care Portal (highly regarded)
