About the role
We are seeking a dedicated Support at Home Care Partner to join our expanding Support at Home team in a full-time capacity. This role is ideal for an individual with a passion for aged care and prior experience in coordinating Home Care Packages.
As a Support at Home Care Partner, you will play a pivotal role in delivering person-centred case management services to our clients. You will work closely with the Support at Home Manager, Nurses, and Support Workers, providing guidance, support, and training to ensure high-quality care outcomes. Your caseload will include clients across various package levels.
This position operates under a flexible, hybrid work model, with a blend of working from home and from our Chermside office. You will also spend part of your time visiting clients in the North Brisbane area to support their needs and maintain a strong connection with the community.
Specific tasks
- Act as the key contact person and advocate for clients in facilitating their Care Plan
- In partnership with the client, develop a goal-focused and consumer-directed Care Plan
- Ensure client needs and preferences are provided within their allocated budget.
- Organise client services and report any issues that arise to the Support at Home Manager.
- Liaise with referring agencies, assessment services, local health providers, and community services.
- Provide day to day support advice and assistance to Support at Home Manager, nurses and Support Worker's.
- Facilitate and conduct training for SWs engaged to provide care services.
- Perform any other reasonable duties as directed by Support at Home Manager.
- Identify additional resourcing needs and provide recommendations to the Support at Home Manager.
Skills & Experience
- Diploma in Aged Care, Community Care and/or Case Management desirable
- Minimum Certificate IV in Aged Care, Community Care and/or Case Management with relevant 2-3 years experience being highly regarded
- Working knowledge of the Aged Care Act and Principles & Aged Care Standards
- Ability to develop, track and understand budgets and financial information
- Previous experience in a care coordination role
- Ability to develop, track and understand budgets and financial information.
- Experience in conducting risk assessments.
- Current National Police Check
- Current CPR & First Aid
