The Opportunity
Support Pathways is a dynamic organisation offering a holistic approach to disability support, providing Psychology, Occupational Therapy, Support Work and Support Coordination services to participants across Victoria.
We are seeking an experienced and motivated NDIS Support Coordinator to join our client's team.
This position will work closely with NDIS participants, their families, and other service providers to ensure the effective implementation of NDIS plans. The goal of this position will be to build participant capacity, enhance independence, and support them in achieving their goals through coordinated and tailored services.
This is a part time position with a minimum of 24 hours per week, with capacity to increase hours up to 38 per week as required.
The Role
As the Experienced NDIS Support Coordination/Psychosocial Recovery Coach, you will:
- Coordinate and manage the implementation of NDIS plans, ensuring participants receive the services and supports they need.
- Build and maintain strong relationships with participants, service providers, and community organisations.
- Monitor and review participant progress, adjusting plans and supports as needed to meet changing goals and needs.
- Provide participants with information, advice, and support to navigate the NDIS system.
- Assist in resolving any issues or barriers participants may encounter in accessing services.
- Maintain accurate and up-to-date records of participant interactions, progress, and service delivery.
- Ensure compliance with NDIS guidelines, safeguarding participant rights and maintaining the highest standards of care.
- Monitor and schedule Support Team Meetings.

About You
To be successful in this position, our ideal candidate:
- Minimum of 12 months' experience as an NDIS Support Coordinator.
- Comprehensive understanding of the NDIS framework, including NDIS principles, standards, and the Code of Conduct.
- Relevant qualifications in Disability, Social Work, Community Services, or a related field.
- Familiarity with Halaxy or similar practice management software is highly desirable.
- Is proficient in all Microsoft Office applications; experience with Xero is highly regarded.
- Knowledge of Flowlogic (or similar case management software) will be highly regarded.
- Currently holds, or is willing to obtain, a NDIS Screening Check, Police Check, CPR certificate, First Aid certificate and Working With Children Check (WWCC).

- Proven experience coordinating complex cases, particularly with participants who have psychosocial disabilities or complex medical conditions.
- Strong interpersonal and communication skills, with the ability to build rapport and work effectively with participants, families, and service providers.
- Excellent organisational and time management skills, with the ability to prioritise tasks and manage multiple participants simultaneously.
- High attention to detail and accuracy.
- Able to work independently and collaboratively within a team.
