Family Support Worker
Pay Scale: Full time: $77,000 to $80,000 plus superannuation
The Role: Ultimate Aged Care is a leading allied health company that specialises in providing comprehensive Allied Health services and Aged Care services. The role of Family Support Worker will be responsible for assisting our qualified team of health professionals in assisting our clients with their services.
With a growing participant base and a team of highly skilled and qualified health professionals, you will work to support both sides of our business in ensuring that our clients are afforded the most quality and most suitable domestic, healthcare and community support.
What We Offer:
This is a full-time role requiring you to work 38 hours per week.
Flexible working Hrs: Working hrs are flexible and cater to your personal needs. We are open Monday-Friday 9am-7pm and Saturday 8am-2pm.
Hybrid Role: The role is hybrid, it is a combination of in-clinic and offsite work.

Key Responsibilities:
·Reviewing the individual needs of our clients and planning and developing the domestic, healthcare and community support programs relevant to their circumstances.
·Consistently monitoring the progress of our clients development in conjunction with their programs and reporting on this to all relevant stakeholders.
·Reviewing relevant My Aged Care and NDIS plans and preparing submissions for renewal or review of the plans to relevant agencies.
·Maintain accurate records and notes for all clients and support workers.
·Coordinate and supervise the support workers responsible for engaging with the clients as part of their domestic and community support.
·Liaising and assisting our nurses, podiatrists, dieticians, psychologist, social workers, speech therapist, occupational therapists, physiotherapists, welfare workers and exercise physiology and medication support staff with client programs, progress and healthcare services.
·Ensure clients are satisfied with their support workers and healthcare professionals and the services and programs they are being provided.
·Conducting interviews with our clients in order to understand their circumstances, needs, preferences and feedback to existing and proposed programs and services.
Qualifications and Skills:
- Diploma or higher in health, disability, community services or equivalent qualification.
- Minimum 1 year of full-time relevant experience.
- Strong communication and interpersonal skills.
- Ability to work effectively within a multidisciplinary team.
