Elevate You Australia (EYA) is a small, personable, 1:1 School Leavers Employment Support Business. EYA are NDIS registered with five different locations across Melbourne. EYA actively services young adults and adults with disabilities and assists them to reach their future independent and employable life goals. EYA connects people to meaningful employment, establish pathways, manages traineeships and apprenticeships, conduct capacity building coaching, and partners with barriered people to create training and employment opportunities.

This role will suit a person that enjoys mentoring, coaching, knowing how to listen, building someone's confidence and capacity in life, and how to motivate, and guide people through transitional periods in their life. Our participants are referred to us directly from their SDS, Support Coordinators, Youth Justice system, Local Area Coordinators and word of mouth.

Job description

We are seeking a full-time, highly motivated, and compassionate Disability Case Manager/mentor/coach to provide support to individuals with National Disability Insurance Scheme (NDIS) School Leaver Employment Supports (SLES), Employment assistance, or Finding and keeping a job funding. As an EYA Case manager, you will be responsible for developing and implementing individualised support plans for participants, transitioning from high school to future adult independent life. There is a strong focus on coordinating and monitoring their progress, and ensuring they achieve their employment and vocational goals.

Location – Dandenong.

Main duties and responsibilities

Core objectives and responsibilities include:

  • Mentoring clients to encourage a level of competence and independence with their employment and training goals.
  • Conduct initial assessments to determine participant's needs, abilities, and goals.
  • Develop and implement individualised support plans that are tailored to each participant's specific needs and goals.
  • Work collaboratively with participants, their families, support workers, and other stakeholders to ensure effective delivery of services.
  • Provide ongoing guidance and coaching to participants and their families throughout their service.
  • Identify and address any barriers to employment or independent living and develop strategies to overcome these barriers.
  • Maintain and monitor accurate and up-to-date records of participant progress and any relevant documentation required by the NDIS.
  • Ensure compliance with NDIS guidelines and requirements, involving report completion and submission where required for funding purposes.
  • Assisting with group activities or excursions – generating, facilitating, and adapting the activity.
  • Liaise with various stakeholders to raise awareness and advocate for the participants needs
  • Engagement and referral to beneficial services which will enhance the participants life and goals.
  • Develop training and mentoring sessions to develop soft skills such as financial knowledge, independent living skills, employment nuances and social etiquettes etc.

About You/Requirements:

  • Bachelor's degree in a relevant field, such as Community Services, psychology, an Allied Health/Teaching related field, disability studies, or social work is preferred. A minimum of Certificate IV in Mental Health, Disability, Health, Therapies, or Case Management will also be considered.
  • A minimum of 2-3 years of experience in disability support services or case management, with experience working with NDIS participants preferred. Experience in SLES highly favoured.
  • Experience and proven knowledge of working with diverse cultures and communities, including Deaf, Autism, and indigenous.
  • You must hold a current driver's license, working with children check and NDIS workers screening check.
  • Excellent communication skills, with the ability to develop rapport and build relationships with participants, families, and other stakeholders.
  • Ability to develop and implement individualised support plans that are tailored to each participant's needs and goals.
  • Strong organisational skills, with the ability to manage multiple priorities and competing demands.
  • Proficiency in Microsoft Office and database management software.
