About the role
Including You is seeking a passionate individual to join our team as an NDIS Support Coordinator. The successful applicant will be responsible for delivering Support Coordination to our participants and/or their families
As an NDIS Support Coordinator, your responsibilities involve understanding participants' needs, preferences, aspirations, and collaborating closely with them, their families, and support networks to craft tailored support solutions. Your role will entail working closely with participants to effectively utilise their NDIS-funded supports, connect them with appropriate service providers, allied health professionals, and monitor their progress towards achieving their plan objectives and goals. You will be committed to promoting inclusivity, informed decision-making, independence, and enhancing the quality of life for all participants.
The position is accountable and reports to the relevant Regional Manager for the efficient, safe, and effective delivery of a range of tasks within predetermined budget, quality, and time constraints; and for working collaboratively with others to continually improve Support Coordination services and uphold our values. The NDIS Support Coordinator is responsible for ensuring that each component of the participant's NDIS Plan is implemented.
Reporting to the Regional Manager - you will:
- Work with NDIS participants, families, and other stakeholders to implement, monitor and support the review of NDIS plans
- Achieve targets as identified by the company and the Regional Manager, in line with the strategic business plan and guidelines as set out by the NDIS
- Support implementation of all supports in the plan, including informal, mainstream and community, as well as funded supports
- Connecting participants with both formal and informal supports to improve their quality of life
- Ethical, customer-focused practice to provide support to the participants to achieve their goals
- Identify and coordinate appropriate service providers and supports for participants, including coordinating the implementation of service agreements
- Provide ongoing support and guidance to participants and their support network to ensure that goals are achieved, and services are delivered in accordance with the participant's plan.
About you
You are a highly motivated individual with some knowledge (preferred) of the NDIS system and processes, excellent verbal and written communication skills, teamwork, high-level organisational skills, and a demonstrated ability to utilise person-centered practices.
To be successful in this role, you require:
- A solid knowledge and understanding of the National Disability Insurance Scheme (NDIS) and specifically the function of NDIS Support Coordination, is essential.
- Experience in utilising a range of community-based supports and methods to achieve participant goals.
- A minimum of 1 year of experience in NDIS Support Coordination is required.
- High-level knowledge and skills in coordination of support to assist people with a disability in their community or their home.
- A minimum of 6 months experience in disability support or community services is required.
- A solid understanding of quoting, budgeting, and financial procedures with strong financial acumen.
- Work autonomously and as part of a small team, taking responsibility for assigned tasks and actively contributing to team meetings and planning sessions

This is hybrid role which is 2 days in the office and 3 days working from home (first 3 months, 5 days per week in the office).
An NDIS Workers Screening Check and Working with Children Check are mandatory requirements of this role. If you do not already hold these, you will be required to obtain them prior to employment.
Qualifications
Cert IV, Diploma or Degree in Community Services, Disability, Mental Health, Psychology, Nursing, Individual Support or similar preferred but not essential
Experience in NDIS Support Coordination is preferred
