About the Service

We are excited to launch a new in-reach rehabilitation model of care for acute stroke patients at Box Hill Hospital. This innovative service aims to optimise rehabilitation access by providing early assessment and intervention for patients who are awaiting transfer to rehabilitation, requiring rehab but not yet ready for subacute care, or those who can be discharged directly home with support. Working in partnership the award-winning acute stroke multidisciplinary team, the in-reach team will enhance patient flow and support high-acuity care by commencing timely, evidence-based rehabilitation interventions.

About the role

An exciting opportunity is available for an enthusiastic and highly skilled Occupational Therapist to join Eastern Health, working in this newly established team. This position will be involved in providing occupational therapy assessment and rehabilitation interventions, and supporting complex decision making and effective discharge planning, with this varied and rewarding caseload. The successful applicant will take a leadership role within the team, include providing supervision to junior staff and students and leading projects. They will work collaboratively with the allied health team to provide and lead excellent patient care.

About you

To be successful in this role you should possess:

  • AHPRA Registration

  • Demonstrated clinical expertise in providing occupational therapy for stroke patients

  • Experience in providing rehabilitation interventions to stroke patients

  • Well developed leadership skills

  • Excellent communication and organisational skills

  • Ability to be flexible and change work priorities as required in a dynamic service

  • A strong commitment to high quality, evidence based care

  • Strong commitment to collaborative interdisciplinary teamwork and patient centered practice

Healthcare workers are strongly recommended to follow COVID vaccination recommendations provided in the Australian Immunisation handbook (based on ATAGI advice). Seasonal vaccination against influenza is a mandatory requirement of this role and an offer is conditional on this being up to date prior to employment.
