Sunhubs Care Services is seeking experienced Disability Support Workers for permanent Full-Time employment to support our clients around the Sydney Metro region.

$78000-$88000 p.a including superannuation

POSITION SUMMARY

The Disability Support Worker is responsible for implementing training and support services aimed at maximizing the living, employment, social, and recreational skills of people with a disability. Community inclusion, independence, decision making, and personal choice should be promoted through these supports, with a focus on individual needs.

RESPONSIBILTIES AND DUTIES

● Support aged and disabled persons to participate in age appropriate activities and tasks including employment, in-home support and Social/community access.

● Provide direct care assistance to each client in accordance with their service plan and as directed.

● Assisting clients with high level daily personal care, hygiene, dressing, and grooming and support as per their Lifestyle Service.

● Preparing and serving meals and assisting with feeding where required.

● Transporting clients to appointments, activities, and community events including but not limited to shopping and run errands as required.

● Assisting clients with mobility and physical transfers.

● Providing social, emotional, and companionship support.

● Undertaking light household cleaning and laundry duties.

● Monitoring health and wellbeing and reporting concerns to supervisors or medical staff.

● Work in accordance with the medication administration policy, work health and safety procedures and client right's policies including privacy and confidentiality.

● Ensure completion of appropriate documentation by following care plans, recording relevant information in client notes and communication books, and ensuring all records are sighted by the Manager.

● Participate in the Lifestyle Service Planning process for individual clients and monitor outcomes.

● Work with clients, family/ carers, relevant case managers and other professionals to support the client's individual goals.

● In conjunction with other members of staff assist in developing, implementing and evaluating training and behaviour programs to attain identified individual goals.

● Ensure that all programming undertaken is appropriately maintained, documented and reviewed.

● Assume the role of key worker for designated clients, including the preparation of a monthly report on client progress.

● Liaise extensively with other staff to ensure the overall smooth and consistent running of the service.

● Actively contribute to a team environment through open communication, participation in regular staff meetings, planning processes, and policy review.

● Provide back-up and support to other staff as necessary.

● Arrange social activities.

● All policies and procedures are followed.

● Records demonstrate that clients have received correct medical and dietary intakes as prescribed by medical practitioners and as per service policy and procedures.

● A high level of communication with the Manager is maintained, including advising of any potential risks to clients or the organisation.

● Sufficient notice (as much notice as possible) is provided for all rostered shifts you become unable to work.

ADDITIONAL SUNHUBS – SPECIFIC DUTIES

· Ensure compliance with Sunhubs' Code of Conduct requirements.

Procedure regarding the collection, storage, use of personal and health information.

· Uphold Sunhubs' values of inclusion, diversity, and respect in all interactions.

· Staff that deliberately breach Sunhubs dress code may be subject to disciplinary action

● May live in with the person if needed.

● Provide 1: 1 or small group training and support in response to individual client needs

● Being adaptable to changing circumstances and being able to prioritise work.

● SUNHUB does not accept any form of aggressive, threatening or abusive behaviour towards its staff by other staff, clients or stakeholders.

● Internet and email are provided to staff members is for genuine work-related purposes.

● Use of SUNHUB communication and information devices are for work purposes only.

● Staff must not share their passwords with another staff member.

REQUIRED CRITERIA/ QUALIFICATIONS

· NDIS Worker Screening Check

· Paid Working With Children's Check

· Current First Aid and CPR certificate

· Police Check

· CV

· Current driver's Licence

· 100 points of ID (Must include proof of Aus Citizenship and or a valid Visa)

· Copy of Car Insurance Policy (Comprehensive)

· ALL 4 Modules on NDIS Quality and Safeguards Commission( Worker Orientation Module - Supporting Effective Communication - Supporting Safe and Enjoyable Meals - New Worker NDIS Induction Module

· Completed Infection Control and Hand Hygiene Modules

EXPERIENCE

Previous experience and or Certificate III/IV in Individual Support (Disability/Aged Care) or equivalent is desirable, with demonstrated ability to provide personal care, community access support, and assistance with daily living activities.
