We deliver a wide range of inclusive and responsive services, with a strong focus on reaching disadvantaged and vulnerable populations in the Inner North and Inner North-Western suburbs. About the Role

We are seeking a passionate and experienced NDIS Support Coordinator who can bring initiative, empathy, and expertise to this vital role. You will support participants to understand and implement their NDIS plans, build their capacity, and achieve their goals through meaningful connections with services and supports.

Hybrid role after successful completion of probationary period.

Skills & Experience

To be successful in this role, you will have:

· A relevant tertiary qualification in Social Work, Human Services, or a related field.

· At least 2 years' experience as a Support Coordinator, particularly in mental health and psychosocial disability.

· Solid understanding of the NDIS legislative framework and Support Coordination processes.

· The ability to work independently and collaboratively within a team environment.

· A demonstrated ability to assess, plan, implement, and evaluate innovative and flexible supports for individuals in a community context.
