As a registered NDIS provider, we deliver mental health and disability supports designed to empower people to live independently, connect socially, and achieve their goals.

**About the Role**

We are looking for compassionate **Community Access Workers** to join our Mental Health & Disability Programs team. In this role, you will provide flexible, person-centred support for people living with disability or psychosocial challenges.

You’ll help participants with daily living, build independence, and support their participation in the community.

**Key Responsibilities**
- Provide outreach and in-home support tailored to participants.
- Assist with activities such as shopping, meal preparation, bills, domestic tasks, and prompting with personal care.
- Support access to community, education, employment, and recreation opportunities.
- Encourage capacity building and personal goal achievement.
- Complete accurate shift notes and administrative records.
- Build positive relationships with participants, families, colleagues, and external providers.

**About You**

You are empathetic, reliable and motivated to make a difference. You’ll also bring:

- Certificate III in Disability, Individual Support or equivalent (minimum).
- Experience in the disability sector.
- Current First Aid & CPR certification.
- Open QLD driver’s licence + reliable, insured vehicle.
- Blue Card & Disability Worker Screening Check (or ability to obtain).
- Double COVID-19 vaccination status.
- National Police Check.
- Desirable:_ Experience with strength-based and capacity-building approaches.
