About the role:
As a Senior Personal Carer, you'll conduct initial client intake and screening assessments and developing and reviewing individualised care plans with a client-centered approach while providing quality care based on clients' needs.
- Conduct thorough initial screening assessments, developing care plans for personal self-care, general house cleaning, social support, respite, transport and shopping assistance.
- Management of client information in CRM & CMS ensuring all details and case notes are kept up to date
- Engage in ongoing care discussions, planning, assessment, reviews, and evaluating goals and outcomes in consultation with the care team and as required.
- Spending your time on the road completing scheduled visits, interacting one-on-one with clients within their homes supporting with daily activities
- Providing care in accordance with their individualised Care Plan
- Supporting with community based recreational and social activities
- Transportation and assistance with shopping and appointments.

Role requirements:
- Certificate III Aged Care, Individual Support or an equivalent qualification - Essential
- Current Australian Driver's Licence (Green Provisional minimum) - Essential
- Roadworthy & insured (Third Party Property minimum) vehicle to use for work- Essential
- Experience with technology incl. smartphones, tablets, laptops etc
- Confidence to satisfy the relevant probity checks required by legislation or policies
