About the Role:
As a Social Worker (AHP2), you will:
- Provide high-quality assessment, care coordination, and treatment services for consumers of community mental health programs.
- Work collaboratively with a multidisciplinary team to deliver integrated, goal-directed care.
- Support consumers and families through discharge planning, psychosocial interventions, and access to housing and community supports.
- Participate in clinical reviews, team meetings, and quality improvement initiatives.
- Contribute to service development, research, and training of students.

- Experience in mental health or related clinical setting
- Competence in mental health assessment, care coordination, and treatment planning.
- Ability to manage complex cases and work independently when required.
- Strong communication and interpersonal skills to engage consumers, families, and stakeholders, which will enable you to successfully problem solve, resolve conflicts and negotiate, including the ability to engage in difficult conversations, whilst building and maintaining relationships
- An ability to effectively manage multiple, diverse and competing priorities and demonstrated resilience within a stressful and complex environment
- Excellent time management and organisational abilities.
- Commitment to continuous learning and quality improvement.
- Ability to work effectively within multidisciplinary teams

NALHN Community Mental Health Services provide specialist acute and recovery-oriented individualised services for people with lived experience of mental illness. Services include psychiatric outpatient and medication clinics, care coordination, individual and group therapy for people aged 18 to 65. Community residential and group rehabilitation programs are provided in a range of settings.

Lyell McEwin Hospital – The major hospital in the north and north-east for emergency care, complex and multi-day surgery, medicine, obstetrics, paediatrics and outpatient services.
