About Us
Ultimate Health Aged Care is a well-established provider of aged care and support services, dedicated to enhancing the lives of elderly Australians through compassionate, client-centred care. As part of our continued growth, we are seeking an experienced Client Coordinator to join our team at our Greystanes facility.
Position Overview
We are looking for a skilled and detail-oriented Client Coordinator to review and manage the activities, services and care for clients individual needs. This role plays a crucial part in ensuring compliance, quality, and the smooth execution of service and government agreements across the organisation.
Key Responsibilities
- Review, negotiate, and prepare the My Aged Care plans between our clients and the Australian government on behalf of the organisation.
- Manage and maintain plan documentation and associated records.
- Liaise with internal stakeholders and external contractors to ensure client's plans are being executed accordingly.
- Work with our domestic support, healthcare and community support stakeholders to ensure plan objectives are being met and adhered to.
- Advise senior coordinator and management on plan and stakeholder compliance and feedback.
- Monitor the performance of all stakeholders and contractors, including aged care workers and report on variations and compliance.
- Prepare reports, submissions, and documentation for management and regulatory purposes to the Australian government.
- Collect and analyse data for plan management and variations.
Requirements
- Relevant qualifications in business, contract management, law, or a related field.
- Minimum of 1 year experience in a similar role.
- Strong knowledge of My Aged Care systems and procurement procedures.
- Excellent written and verbal communication skills.
- Strong attention to detail and problem-solving skills.
- Ability to manage multiple tasks and meet deadlines.
Desirable
- Previous experience in aged care, healthcare, or community services settings.
- Familiarity with Australian industry regulations and compliance standards.
