Job Description

As a Case Manager you will be required to case manage an allocated caseload of participants providing integrated case management. This will include actively engaging participants with the program, supporting participants to find sustainable employment and improving participants capacity to work by stabilising non-vocational barriers and building vocational skills and experience.

Work in a Continuous Quality Improvement cycle to ensure case management practices are aligned with contractual obligations;

  • Be a positive and motivational mentor encouraging participant engagement with the Program;
  • Undertake assessments and goal setting to strengthen a participant's ability to find and sustain employment;
  • Provide individual coaching and support to a diverse range of Participants, which may include career planning, resume writing, job

search support, improving employability skills, addressing personal issues, as well as improving skills, training, and education;

  • Facilitate access and referral to internal MEEDAC projects and services as well as external specialist organisations;
  • Secure sustainable employment opportunities through Participant referral to internal and external job vacancies as well as

advocate to the MEEDAC Employment Support Unit (ESU) on a participant's behalf to find work experience or employment;

  • Assist with Post Placement Support and Employer Engagement when required;
  • Document all Participant contacts in a timely and accurate manner and undertake all required administrative duties utilising

internal MEEDAC designed tools;

  • Assist with relief case management and activity delivery when required;
  • Participate in staff meetings and staff development training;

REQUIREMENTS

  • Must hold a valid Australian Driver's Licence;
  • Must be an Australian Citizen;
  • Appointment will be subject to a satisfactory Australian Police Clearance and other checks as required;
