Sky High Healthcare is the Social Enterprise of Jack's Butterflies, who is a registered charity with the ACNC, an NDIS registered service provider and a Rare Voices partner providing advocacy and support for children and their families affected by a life-limiting Rare Diseases.

At Sky High Healthcare, we employ Cleaners, Support Workers, High Intensity Support Workers, Mental Health Support Workers, Enrolled Nurses and Registered Nurses providing in-home support, community access, short term accommodation and respite care.

About the Roles

SIL Team Leader

We are currently recruiting for a Supported Independent Living (SIL) Team Leader to oversee the day-to-day operations of one of our supported homes.

This role includes a combination of direct support work (3-4 shifts per week with the client) and administrative duties (1-2 shifts per week), including completing documentation, coordinating support plans, and liaising directly with your manager to ensure a high standard of care.

Experience in a similar leadership role within the disability or community support sector will be highly regarded. If you're passionate about making a meaningful difference and have strong organisational and communication skills, we'd love to hear from you

Disability Support Workers

Additionally, we are looking for 8 people to join our incredible team of passionate, dedicated and hard-working Support Workers. The number of hours per week available is dependent on your availability. There are a range of shifts spanning over a 24-hour period, to be successful for this role, you must be willing and able to assist with inactive sleepover shifts when required. Your day-to-day duties will consist of supporting the individuals with all activities of daily living, community access, transportation and supporting to appointments.

Responsibilities

Not every client has the same requirements and each client has their own way of doing things. The biggest asset you can posses, is the ability to not assume you know how to do it from your own experience, but the ability to use your knowledge as a starting base to learn and adapt to your new clients.

Team Leader Responsibilities

  • Oversee all day-to-day operations within the supported living home.
  • Support participants by coordinating, booking, and attending appointments as required.
  • Manage and complete regular stocktake and household supply requirements.
  • Ensure all support staff are completing shift documentation accurately and in a timely manner.
  • Promote consistency in support delivery by guiding the team to follow unified, person-centred approaches.
  • Provide weekly progress updates to your direct manager, including timely reporting of any changes in participant needs or escalation requiring allied health involvement.

Support Worker Responsibilities

  • Providing individualised behaviour support in line with the Behaviour Support Plan
  • Provide personal care to individuals with disabilities, including assistance with showering toileting, dressing and grooming.
  • Assist with in-home support, including general housework, cooking and cleaning.
  • Utilise manual handling techniques, to support individuals with mobility impairments.
  • Maintain accurate documentation and reporting of client progress and behaviours.
  • Ensure medication is taken on time and correctly and monitor medication schedules.
  • Facilitate community access, including accompanying clients to appointments, activities and events.

Requirements

  • Paid Working with Children Card (Blue Card)
  • NDIS Worker Screening Check (Yellow Card)
  • First Aid Certificate
  • CPR Certificate
  • Worker Orientation Module Certificate
  • Available on weekends and active overnights
  • Previous experience working in disability support is preferred.
  • Reliable vehicle safe for transporting clients
