Care By Phoenix is a dynamic, mid-sized NDIS provider offering high-quality Support Coordination, Daily Living, and Social & Community Participation services as well as 2 SIL homes. We also deliver tailored day programs and provide respite options to support participants in achieving their goals.

We're now looking for a passionate, experienced Support Coordinator to join our Wynnum office and help us continue delivering exceptional, client-centred services.

About the Role

As an Experienced Support Coordinator, you will play a vital role in empowering NDIS participants to achieve their goals and live fulfilling, independent lives. You will work closely with clients, families, and service providers to ensure every participant receives the right level of support.

Key Responsibilities:

  • Deliver general support coordination in line with NDIS Practice Standards.
  • Manage Change of Situation requests and ensure smooth transitions.
  • Maintain NDIS compliance and prepare accurate, timely reports.
  • Coordinate and oversee rostering for support services.
  • Provide regular, professional client contact to ensure needs are met.
  • Support clients through plan reviews, plan implementation, and goal setting.
  • Assist with budgeting and monitoring of client plans across all services.

About You

  • An experienced Support Coordinator with a proven track record in the NDIS sector (essential).
  • Knowledgeable in NDIS processes, compliance, and reporting requirements.
  • Skilled in problem-solving and thinking outside the box to achieve client outcomes.
  • A strong communicator who values collaboration with participants, families, and support networks.
  • Organised, with experience in rostering, plan management, and budgeting.
  • Passionate about making a difference in the lives of people with disability.
