Sunhubs Care Services is seeking experienced female disability support worker specialising in mental health to work across various shifts, supporting a female client in the community and at home.

Job description

Experience with mental health and behaviours to assist our client with de escalation strategies as per her behavioural support plan.

Excellent communication with customers using whatever means of communication is appropriate for each individual

Consult with customers and take account of their wishes in all matters relating to their care with the proviso that Duty of Care and WHS issues must also be considered

Implement and review of Individual Program Plans

Record the progress of each customer in line with relevant documentation

Provide transport for customers or assistance to use public transport

Provide physical assistance as required. This may include: Assistance with meals

Assistance with personal care when needed.

Medical/special procedures where required

Administering medication where required

Physical assistance to participate in activities

Domestic task duties

Support and actively encourage customer's participation in decision-making about their own program and about the service

Complete documentation as required. This may include:

Customer notes

Shift reports

Learning Logs / Monthly reports

Risk management / Positive behavior support

Medication forms

Timesheets

Attend staff meetings, supervision meetings, training and other relevant meetings as required

Perform the necessary duties to ensure that the premises and vehicles are kept clean and tidy

Contribute to individual service planning and review

Maintain a professional standard of behaviour in keeping with the SUNHUBS' Code of Conduct and Ethics

Work effectively as part of a team and utilize grievance procedures if problems arise

Utilise a Positive Behaviour Support approach when working with customers, to follow Sunhubs' Positive Behaviour Support policy and procedure, and to participate in the development, implementation and record keeping for all related plans as applicable to their role.

In return, Sunhubs will provide all training and mentoring on the job.

Pay: $35.00 – $54.35 per hour

Experience:

  • Disability support: 1 year (Preferred)

Licence/Certification:

  • NDIS worker check (Preferred)
  • CPR/First Aid certificate (Preferred)
  • Driver Licence (Preferred)
