Award: SCHADS Home Care Level 2 (depending on skills/quals) + penalties + km reimbursement

About the role

We're growing our Sydney team and seeking experienced Support Workers to assist NDIS participants living with one or more of the following diagnoses: Schizophrenia, Intellectual Disability, Traumatic Brain Injury (TBI), high/complex behaviours, and Substance Use Disorder (dual-diagnosis).

You'll provide safe, respectful daily support across home and community settings, using trauma-informed practice and Positive Behaviour Support (PBS).

What you'll do

  • Provide person-centred support with personal care, medication prompts, mealtime support, community access, transport and skill-building.
  • Implement Behaviour Support Plans (BSPs), use ABC notes and proactive strategies; escalate and de-escalate safely using approved techniques.
  • Support participants experiencing psychosis, mood dysregulation, memory/processing challenges post-TBI, and drug/alcohol-related risks using harm-minimisation principles.
  • Follow risk/safety plans, complete incident and hazard reports promptly.
  • Record high-quality shift notes, logs and progress reports (clear, objective, NDIS-ready).
  • Work in partnership with families, clinicians (OTs, Psychologists), and coordinators to keep supports consistent and effective.
  • Uphold dignity, choice & control, cultural safety, and professional boundaries at all times.

About you

  • Calm under pressure with strong de-escalation and communication skills.
  • Confident supporting complex behaviours, mental health crises and dual diagnosis.
  • Professional, reliable and team-oriented; punctual with great documentation habits.

Must-haves

  • Driver's licence + reliable, comprehensively insured car (essential).
  • NDIS Worker Screening Check (or ability to obtain).
  • Working With Children Check (paid/employee).
  • First Aid (HLTAID011) & CPR (HLTAID009) – current or booked.
  • Right to work in Australia.
  • Availability for a mix of weekday/weekend and some sleepover/night shifts.

Great to have (highly regarded)

  • Cert III/IV in Individual Support, Mental Health, Disability, Community Services or similar.
  • Training/experience in PBS, MAPA/PMVA/TCI (or other approved behaviour safety programs).
  • Experience with Schizophrenia, ID, TBI, and Substance Use supports in home/community.
  • Understanding of harm minimisation, trauma-informed care, safeguarding and duty of care.
  • Confidence using apps for rosters, logs and incident reporting.
