Mayflower is seeking a Clinical Care Coordinator, who will be responsible for supporting clinical team leaders with clinical reviews including responding, planning, investigating, assessing, documenting and reporting.

The role successfully manages the professional and clinical development of Clinical Team Leader and ensures the delivery of high level and consistent care in line with Mayflower's person centered approach.

About the Role

  • 6 Month contract (with possibility of extension), Full Time - Monday - Friday
  • $ 59.65 per hour + Super, with an option of Salary Packaging
  • Location – Based at our Brighton Aged Care.
  • Flexible Work Arrangements
  • Support from Care Manager and General Manager Onsite

As a Clinical Care Coordinator you will:

  • Support Clinical Team Leaders with clinical reviews including responding, planning, investigating, assessing, documenting and reporting
  • Make sure that all care plans and assessments are up to date and is reflecting the care being provided
  • Ensure the delivery of high level and consistent care in line with Mayflower's person-centered approach
  • Maximise ANAAC by supporting appropriate documentation and identifying opportunities for improvement
  • Ensure Medication Management practice is adhered to by all PCA medication staff, EEN and RN's
  • Ensure resident needs are met by reviewing call bell reports and communicating time delays with employees
  • Respond to care needs of ILU residents as required
  • Review incident reports and action or escalate to the Care Manager

The successful candidate will have:

  • AHPRA Registered Nurse Div 1 Registration
  • NDIS Worker Screening Check
  • Full COVID vaccination
  • Minimum 3 years' experience in a Clinical Management role (Aged Care experience highly regarded)
  • Knowledge and implementation of Aged Care standards
  • Demonstrated knowledge and commitment to the delivery of person-centered care
  • Knowledge and understanding of the diverse needs of older people with disabilities
  • Well versed in medication management, falls management and clinical care
  • Highly developed interpersonal and customer service skills
  • Demonstrated analytical and problem-solving skills
  • Experience in managing and mentoring staff
  • Intermediate computer literacy
