Clinical Care Coordinator – APS6 (Labour Hire Opportunity) 

We are currently seeking experienced and committed Clinical Care Coordinators (APS6) to join a national mental health support program, dedicated to enhancing the wellbeing and mental health outcomes.

This is your opportunity to play a key role in delivering trauma-informed, client-centred care within a multidisciplinary environment. As a Clinical Care Coordinator, you will work in short-term, goal-oriented clinical case management with a focus on engagement, risk management, and positive client outcomes.

Key Responsibilities In this dynamic and meaningful role, you will:

  • Provide high-quality, evidence-informed clinical services including intake, assessment, risk management, brief intervention, and care coordination
  • Support clients with complex mental health and psychosocial needs using strategies such as psychoeducation, motivational interviewing, and distress tolerance
  • Build and maintain partnerships with internal and external stakeholders including community-based mental health services, ex-service organisations, and government agencies
  • Maintain accurate and confidential client records in line with privacy and clinical governance requirements
  • Actively contribute to multidisciplinary case reviews and team meetings
  • Participate in continuing professional development, including clinical supervision, to maintain professional standards and registration
  • Engage in program delivery activities such as group treatment facilitation, community engagement, and service promotion

Skills & Experience We're Looking For

  • Excellent interpersonal skills with the ability to deliver trauma-informed and holistic interventions
  • Experience with client intake, assessment, care planning, and managing complex cases
  • Sound knowledge of mental health service systems and referral pathways
  • Strong written and verbal communication skills, including clinical documentation
  • Ability to manage risk and respond effectively to critical incidents
  • High IT proficiency, including case management systems and Microsoft Office

Mandatory Requirements To be eligible, you must:

  • Be an Australian Citizen
  • Hold (or be able to obtain) a Working with Children Check or Working with Vulnerable People registration
  • Meet one of the following registration requirements:
  • Psychologist, Occupational Therapist, or Registered Nurse with full AHPRA registration
  • Social Worker with current AASW membership
  • Clinical Counsellor with registration at PACFA or ACA Level 3 or 4
