About the Role
We're seeking a compassionate and experienced Clinical Care Coordinator to provide high-quality, evidence-informed case management and clinical support to individuals with complex needs. This role focuses on helping clients engage meaningfully with mental health care services and support systems through short-term, goal-oriented interventions.
As a Clinical Care Coordinator, you'll work one-on-one with clients to identify barriers to access, connect them with the right supports, and promote engagement in ongoing care. You will also liaise with a wide range of internal and external stakeholders to support the broader wellbeing of veterans, defence personnel, and their families.
Key Responsibilities
- Provide clinical case management, risk assessment, brief interventions, and referral services to clients with complex and/or multiple needs.
- Collaborate with internal teams and external service providers to deliver integrated care and facilitate support pathways.
- Maintain accurate and confidential clinical records, care plans, and risk documentation in the Client Management System.
- Participate in clinical supervision, professional development, and continuing education to maintain professional registration and high standards of care.
- Support and/or co-facilitate group treatment programs and contribute to service promotion, community engagement, and evaluation initiatives.
Skills & Experience
To be successful in this role, you will have:
- Strong interpersonal and communication skills with the ability to engage and support clients in distress.
- Demonstrated experience in trauma-informed, client-centred practice.
- High-level organisational and documentation skills, including the ability to maintain clinical records and assessments.
- The ability to remain calm under pressure and respond effectively to critical incidents.
- Proficiency in using IT systems, clinical software, and Microsoft Office.
Mandatory Requirements
- Must be an Australian Citizen (in line with federal government requirements).
- Ability to obtain and maintain a Baseline AGSVA Security Clearance.
- Must hold or be able to obtain a Working with Children Check or Working with Vulnerable People registration (depending on jurisdiction).
Relevant professional registration or membership with one of the following:
Full AHPRA registration as a Psychologist, Occupational Therapist, or Registered Nurse, OR
- Full membership with the Australian Association of Social Workers (AASW), OR
- A Master's level Counselling qualification with PACFA (Registered Clinical Counsellor) or ACA Level 3 or 4 registration.
