Galangoor works with the local Indigenous community and relevant key stakeholders to increase access to culturally safe, quality, coordinated and comprehensive primary health care services. We focus on healthcare across the life span, including the management and reduction of chronic disease, to address the health and social inequities of Aboriginal and Torres Strait Islander communities of the Fraser Coast Region.
About the Opportunity
Galangoor Duwalami Primary Healthcare Service is seeking a NDIS Community Connector to join us in creating Healthy Living, Healthy Communities and a Healthy Futureon Queensland's fabulous Fraser Coast.

About the Role
As the NDIS Community Connector, you'll be a key support for Aboriginal and Torres Strait Islander people, and their families on the Fraser Coast, providing support to navigate the NDIS. This role will suit someone who has experience in NDIS and is looking to expand their skills into a community connection and engagement role. This unique and newly formed position allows you to directly impact the growth of our NDIS team, where your NDIS experience and knowledge will be highly regarded.

Galangoor Duwalami means "Good Meeting Place" and that's exactly what our clinics in Hervey Bay and Maryborough provide. A place where the region's Aboriginal and Torres Strait Islander community can access a comprehensive suite of quality Primary Healthcare Services to fit all ages and stages of their health journey.
As a Community Controlled medical service established in 2007 and Queensland's second largest ACCHS, Galangoor has two newly purpose-built clinics in Hervey Bay and Maryborough that offer exceptional facilities and are only 30 minutes apart. Our Hervey Bay clinic is centrally located in one of the city's busiest shopping precincts, while the Maryborough clinic is situated in the heart of the Heritage City.

Essentials:
Knowledge, Skills and Capabilities
· Sound understanding of NDIS and operational guidelines
· Commitment to fostering culturally safe, person-centred and trauma-informed practices that empower and support individuals
· Ability to tailor communication to the needs of the individual, families, carers, internal teams and external professionals
· Capability to simplify the complexities of the NDIS, ensuring the information is accessible and understandable for those seeking access, current participants and their families and carers
· Ability to create strong community connections and networks
· Strong understanding of referral pathways in mainstream and community sectors and connecting NDIS applicants and participants to these services
· Good record management, case notes, data collection and reporting capabilities.
Key Requirements:
· Demonstrated knowledge and experience working with the National Disability Insurance Scheme (NDIS)
· High level of confidentiality and ability to handle sensitive information.
· Excellent written and verbal communication skills
· Computer literacy including MS suite
· Strong attention to detail with a high level of accuracy.
Desirable:
· No qualification is required for this role, however qualifications in individual support, community services, healthcare, Aboriginal mental health first aid will be highly regarded.
Mandatory Qualifications:
· Current C Class Drivers Licence (QLD)
· NDIS Worker Screener
· Current Blue Card
· Current annual CPR Certificate
· Current First Aid Certificate (every three years)
· Criminal history checks will be undertaken on the preferred applicant(s). (A criminal conviction or charge will not automatically exclude an applicant from consideration for appointment. If information is received that may exclude you from further consideration, you will be given an opportunity to respond and your response will be taken into account in the evaluation process.)
· Recommended Immunisations as per Galangoor P&P
