CODA Disability Support Assoc. Inc was established in 1989 and is an NDIS registered, not-for-profit organisation. CODA is an organisation that prides itself on the work we do and supports we provide which include - 24/7 accommodation and SIL support; STA and holiday support; Community participation supports; Plan Management and Support Coordination services to adults with a disability living in the extended Brisbane, Ipswich and Bayside areas.

Based in Upper Mount Gravatt, we are seeking a client-focused, Coordination superstar to join our vibrant, hardworking, and committed team as a permanent part-time (30 hours per week) NDIS Support Coordinator, with an increase to hours and transition to full-time as our service continues to grow.

Reporting to the Client Services Manager, you will work closely with clients to help them navigate the NDIS, coordinate supports, and achieve their goals. You'll bring exceptional communication and organisational skills, a strong understanding of the NDIS framework, and a commitment to empowering individuals to live their best lives.

Your responsibilities will be to:

  • Assist participants to negotiate provision of services and supports; monitor the delivery of services and supports and assist participants to understand how their NDIS Plan will fund supported needs.
  • Promote and assist to build an individual's choice and control and assist in preparing participants for plan reviews.
  • Bring a range of resources and an understanding of the scheme and how to optimize best supports for participants.
  • Adhere and work to the Policies of the organisation, the NDIS Code of Conduct and have knowledge of the NDIS Act 2013.

The successful applicant will have:

  • Relevant qualifications or relevant extensive experience in support coordination, case management, disability, social work or community services (or relating field).
  • Knowledge and understanding of the National Disability Insurance Scheme (NDIS) and the disability sector.
  • A range of resources and an understanding of the scheme and how to optimize best supports for participants.
  • Ability to work in a very busy environment and to work both independently and as a part of a team
  • Have excellent communication skills including report writing skills and proficient computer skills
  • Excellent organisational and time management skills and ability to manage own caseload.
  • Valid Australian Driver's Licence
  • A current NDIS Worker Screening Card (or willing to obtain)
  • NDIS Worker Orientation module certificate
