Role Description

This is a full-time on-site role for a Community Support Worker, located in Brisbane, QLD. The Community Support Worker will provide holistic mentorship and support to men facing psychosocial disabilities. Responsibilities include direct engagement with clients, case management, developing and implementing personalized support plans, and facilitating community health activities. The role also involves fostering a supportive and understanding environment where clients feel safe and empowered.

Qualifications

  • Strong Interpersonal Skills and Communication abilities
  • Experience in Community Support and Community Health sectors
  • Proficiency in Case Management
  • Ability to build and maintain authentic connections with clients
  • Capacity to work effectively in an on-site environment
  • Experience with NDIS frameworks is a plus
  • Bachelor's degree in Social Work, Psychology, Mental Health, or related field is preferred
