About the role

As a Clinical Care Partner, you'll play a key role in enhancing the participant experience—supporting health, wellbeing, and continuity of care to help people age well at home. You'll work closely with participants and their support networks to create tailored care plans, deliver person-centred case management, and champion wellness outcomes. If you're passionate about quality care and ready to guide individuals through complex and meaningful care journeys—including restorative and end-of-life pathways—this is your opportunity to lead with heart and purpose.

What will this look like on a day to day basis?

You will be:

  • Conduct clinical assessments and evaluations within professional scope;
  • Maintaining high quality assessment and care planning documentation;
  • Fostering respectful relationships with clients to fully understand their needs and ensure our services are reflective of person centred practice and the strengthened Aged Care Quality Standards;
  • Providing comprehensive case management, ensuring the day to day wellbeing of our clients through case work, advocacy and planning;
  • Liaise with a range of stakeholders including providers, suppliers, carers, supporters, clinicians and multidisciplinary teams regarding appropriate provision of supports; and
  • Monitor and respond to changing needs and emerging risks ensuring care plans reflect identified goals.

What skills and experience do you need to be a Clinical Care Partner?

This position is best suited to someone who:

  1. Has a tertiary qualification in a relevant health-related discipline eg. Bachelor of Nursing
  2. Knowledge and experience in clinical coordination including conducting assessments and evaluations
  3. Is experienced in case management preferably within the home and community care sector with advanced knowledge of person-centred care
  4. Has experience in service development, and driving continuous quality improvement and practice and service excellence aligning to the Strengthened Aged Care Quality Standards
  5. Is relatable and engaging with highly developed interpersonal, communication skills and emotional intelligence with the proven ability to foster relationships and connect with internal and external stakeholders
