Note: This role will be filled at either Health Professional Level 3, Psychologist Level P3 or Registered Nurse Level 2 depending on qualifications and experience, in line with the Enterprise Agreement.
Salary for HP3: $109,543 - $117,878 (Plus 12% Super)
Salary for P3: $120,806 - $131,180 (Plus 12% Super)
Salary for RN2: $110,733 - $117,053 (Plus 12% Super)

About the Role:
- This is a multi-classified position with possibility of appointment at either HPO3, P3 or RN2. Please review 02OEY if you are a Registered Nurse interested in this position. There is only one position available.
The Paediatric Liaison and Navigation Service (PLaNS) provides information, coordination, and navigation for children with complex health care needs aged 0-18 years old. The focus of the service is on improving the child's and family's experiences and health outcomes through information sharing, integration of care across primary, secondary, and tertiary services, and coordination across health and community services. This includes a focus on providing care outside the acute setting with outreach to clients in the ACT community.
Children and their families will be eligible for the service when there is a combination of health care needs across medical and psychosocial domains that require support from multiple providers across the health and community sectors. This will include children who have a shared care arrangement between ACT and interstate health services.
The PLaNS team is comprised of a multidisciplinary team with positions located within a range of CHS service areas. This role will be full-time in the Paediatric Care Navigator role and will undertake assessment from an interdisciplinary perspective and coordination of treatment/therapies and psychosocial supports to ensure the facilitation of timely and safe access.
This role involves liaising and working with all health services involved in the child's care; focusing on improving the quality of care and experience of a child and their family, and on continuous service improvement. The role will contribute to the supervision and training of colleagues and students. The Paediatric Care Navigator will have highly developed professional knowledge of a range of Paediatric conditions with the ability to lead and provide education and supervision to a broad range of staff.
This unique role is an opportunity to build relationships with families and children, and with diverse acute and community services, to anticipate and reduce the challenges that children and young people with complex health needs, and their families, can face. It is an opportunity to ensure the highest quality of care, information and support is provided when families and children need it, to optimise their health and wellbeing and assist them to navigate the complexities of their care.
The role will be a point of coordination with interstate hospitals/health services and work with CHS services and current specialty care coordinators to support individual families and improve the formal networks and pathways for information sharing, coordination, and ED avoidance.
The position will work to develop the clinical scope of the role with the intention to undertake practical clinical support and interventions to support consumers to stay well in the community, and to work directly with outpatient and community services to minimise the need for acute care presentation.
The Paediatric Care Navigator will take on a key role in the ongoing review and development of the service, working collaboratively with colleagues, consumers, and external agencies to contribute to and guide the ongoing evaluation and improvement of this innovative family-centred service.
