Patient Assessor Job Summary:
Here at BridgePoint the Patient Assessor conducts pre-admission patient screenings to determine clinical needs and appropriateness for admission to our Long-Term Acute Care Hospital setting. The Patient Assessor coordinates collection and transmission of clinical and benefits information from referring hospitals back to our Centralized Admissions Office. Identifies specific patient needs for equipment, supplies and consult services. The Patient Assessor role facilitates physician to physician communication as needed. Works closely with patients and their family members to educate them on the benefits of our BridgePoint Hospitals. The Patient Assessor provides territory coverage as needed when assigned Territory Manager is on PTO. Meets with management staff, participates in Marketing meetings and serves on various internal committees, as assigned.
RN, Registered Nurse, LPN, Nurse, Licensed Practical Nurse, LVN, Licensed Vocational Nurse, Respiratory Therapist, RT, RRT, CRT, Respiratory Care Practitioner, RCp
QualificationsEducation: Associates degree in a clinical field or an LPN certificate in lieu of. Bachelor's degree preferred.
Licenses/Certification: RN, LPN, or RT (respiratory therapist) license required.
Experience: Minimum 2 years of acute care experience in a hospital, LTACH, Acute Rehab, or High Acuity SNF that has a ventilator unit.
