Make a Difference Every Day…

As a valued member of our Community Liaison team, you will:

  • Oversee and coordinate the efficient discharge of patients, ensuring continuity of care and safety
  • Monitor and facilitate patient flow across departments to optimise bed availability
  • Apply clinical reasoning to assess and coordinate complex patient care needs, post-discharge
  • Build collaborative relationships with patients, families, GPs, community services, and internal teams
  • Navigate healthcare resources and contribute to efficient business operations and care outcomes

What makes a difference for us?

We're seeking an individual who brings:

  • AHPRA registration as a Registered Nurse
  • Bachelor of Nursing (or equivalent)
  • Minimum 3 years post-graduate clinical experience in a healthcare setting
  • Excellent communication skills with an empathetic, patient-centred approach
  • Strong time management, critical thinking, and decision-making abilities
  • Confidence to work both independently and as part of a multidisciplinary team
  • Understanding of evidence-based practice and care coordination principles
  • Knowledge of health fund models and discharge planning systems (desirable)
