This includes developing individual support plans, coordinating access to housing and support services, and advocating for clients to overcome barriers such as financial hardship, health issues, or family violence.
In addition to direct client work, you will mentor and support junior staff, promote best practice in case management, and provide guidance on complex cases. Collaboration with internal teams and external partners will be key to ensuring integrated and effective service delivery.
