We have a Sub Contractor Assessor paid per assessment position available within our Aged Care Needs Assessment team - Clinical (ACNA )

The opportunity:

Interchange is seeking a knowledgeable, caring and pro-active sub contractor to join our Aged Care Needs Assessment team ( ACNA ) clinical assessor.

Travel is an expectation in this role, which includes driving and conducting face to face assessments in the client's home.

You will be reporting to the Team Leader and must also be able to work autonomously.

The role responsibilities include:

  • Completing face-to-face assessments to determine older Australians' eligibility for support within the Clinical Aged Care Needs Assessment space.
  • Reviewing previously assessed clients.
  • Undertaking client-centred support planning using a wellness and reablement approach.
  • Liaising with referral sources and community support organisations to assist our clients to obtain the home supports they require.
  • Ensuring accurate and timely completion of assessments and documentation, utilising the My Aged Care online portal.
  • KPI based.

Preferred candidates will have

  • Registered Nurse or Allied health professional
  • Current AHPRA registration.
  • ABN
  • Current Public Liability Insurance
  • Completed My Aged Care, Home Support Assessor Training (SOA2) or willingness to obtain
  • Ability to work independently, and as part of a team.
  • Experience in meeting KPI's
  • High level of computer literacy skills
  • Ability to follow instructions and guidelines relevant to the position as well as demonstrated problem-solving capacity.
  • Current Australian driver's licence
  • Own Mobile phone and home internet
  • Own laptop
