Healthy Ageing Community Connector

The Elder Care Connector role will support identified clients to navigate access to appropriate local aged care support services. They will provide advocacy support to ensure clients receive a culturally safe plan assessment.

The AACC will support clients to understand their aged care service entitlements, including My Aged Care registration, contractual arrangements and costs, and will help ensure Elders and their family are well-prepared for plan reviews and outcomes. This role will liaise with aged care Assessors to ensure Elders are well-supported throughout the assessment process.

Underpinning the role are two key work practices: cultural safety and trauma informed practice.

The Elder Care Connector has the primary responsibility of connecting with local Elders, older Aboriginal and Torres Strait Islander people and their families to raise awareness of their aged care entitlements.

The overarching function of the Elder Care Connector is to ensure that support for clients is organised, sequenced and supported to enable effective access to appropriate aged care services both internal and external to the ACCHO.

1. KEY SELECTION CRITERIA

2. Aboriginal or Torres Strait Islander (NB: applicants race is a genuine occupational qualification and is authorised by Section 14 of the Anti-Discrimination Act 1977, NSW)

3. Experience working with Aboriginal and Torres Strait Islander people ad have a strong connection to the community
4. Excellent communication and interpersonal skills, with the ability to build trust with community.
5. Demonstrated understanding of the principles of person-centered care and trauma-informed practice.
6. Understanding of how to assist and support clients to access services, reduce service complexity and confusion.
7. Sound understanding of My Aged Care and operational guidelines.
8. Good record management, data entry & collection and reporting capability.
9. Experience in the community services field with strong community connections and networks across aged care providers, mainstream supports and community sector.
10. Ability to work autonomously and be a self-starter, with a high degree of initiative and problem-solving skills across multiple sectors.
11. Experienced in working in a team environment that is open and transparent, with a person-centered focus.

Remuneration: $35.67  per hour + superannuation
