About the Role:
As a Discharge Liaison Nurse in the Division of Medicine at Canberra Health Services, you'll play a key role in coordinating safe and timely discharges across a range of inpatient wards, including Gastro-medical, Respiratory, Cardiology, Neurology, and Renal. Working Monday to Friday during business hours, you'll collaborate with multidisciplinary teams to ensure patients move confidently from hospital to home or community care.
