North Western Melbourne Primary Health Network (NWMPHN) is seeking an experienced Program Officer to support primary care providers to deliver quality care and implement relevant health reforms.

  • Remuneration approx. $73, $88, % superannuation

The Program Officer, Quality Improvement will play a pivotal role in supporting general practices and allied health to deliver high-quality care through the implementation of quality improvement initiatives and relevant health reform projects. This is achieved through capturing innovation and learnings for translation into other programs and activities. This role requires a proactive, planned, and accountable approach to enhance practice performance and patient outcomes.

As the primary liaison between primary care and the PHN (Primary Health Network), the Program Officer will serve as the key relationship manager. You will work collaboratively with stakeholders, fostering strong, ongoing partnerships to drive the successful execution of quality improvement activities and support continuous improvement across the sector.

We are North Western Melbourne Primary Health Network (NWMPHN), Victoria's largest independent, locally governed and run, not-for-profit PHN, dedicated to improving primary health care in our community.

What are PHNs?

PHNs have been established with two key objectives:

  • increasing the efficiency and effectiveness of services for community members, particularly those at risk of poor health outcomes
  • improving coordination of care to ensure people receive the right care in the right place, at the right time

A typical week may include:

  • Maintaining high performing working relationships with internal and external stakeholders, to deliver outcomes within services and programs
  • Plan, coordinate, facilitate, and deliver quality improvement projects and activities with general practice and other NWMPHN staff
  • Coaching practice teams to use and embed quality improvement tools and methods within general practice
  • Support contract management for service providers, education and training providers or general practices
  • Collaborating and integrating activities across the organisation to achieve shared goals and enhance general practice and allied health engagement and relationships

We are looking for someone who has:

  • Qualifications or relevant experience in public or not for profit sector, primary health care or health care
  • Project management experience - experience in scoping, planning, monitoring, and reporting on projects
  • Demonstrated capability for quality written communication, including the preparation of external communications and reports to a professional standard
  • Demonstrated relationship management skills including the ability to develop and maintain strong collaborative partnerships and to work with all levels within the workplace
  • Demonstrated understanding of using and interpreting data to guide improvement
  • Ability and competence in the use of technology and a variety of software systems, including competency with MS Office (Excel, Word, PowerPoint)
