Co.As.It. is seeking a Coordinator - Case Management  for immediate start.

The role of the Coordinator - Case Management is to provide quality care to all clients. This will ensure the continued success of the community care programs at Co.As.It.

This  role oversees Home Care Packages aligned with consumer-directed care principles, ensuring that older Australians—especially those from CALD backgrounds—receive person-centred, flexible, and culturally appropriate support to age independently at home.

SKILLS, KNOWLEDGE AND EXPERIENCE REQUIRED

  • A degree in Social Work, Social Welfare, Nursing, or equivalent experience in community or aged-care settings.
  • Strong understanding of issues affecting frail older adults, including those living with dementia or from culturally diverse backgrounds.
  • Staff supervision experience and exceptional interpersonal, organisational, and communication skills.
  • A current Driver's Licence and fluency in English (Italian language skills are desirable).
