Latrobe Regional Health (LRH) is looking for Transitional Care Coordinator to join our growing team on a full-time on a permanent basis.
This pivotal role focuses on facilitating seamless transitions in care for consumers across the mental health division, with an emphasis on supporting safe and coordinated discharge from bed-based to community-based services.
As the Transitional Care Coordinator, you will play a vital role in improving health outcomes, reducing post-discharge risks and readmissions, and enhancing consumer wellbeing through personalised, collaborative care planning and follow-up.

The role of the Title is:
- Coordinate and oversee discharge and transition planning in line with LRH protocols.
- Initiate discharge planning within 72 hours of admission and collaborate with inpatient and community clinicians.
- Engage consumers, families, carers, and nominated supporters to develop and implement robust transition plans.
- Facilitate multidisciplinary care and discharge planning meetings.
- Ensure timely follow-up contact with consumers within 72 hours post-discharge.
- Work closely with the Lived Experience Workforce to promote person-centred and trauma-informed care.
- Monitor outcomes, track readmission data, and contribute to quality improvement initiatives.
About you
Ideal candidates for this role will have:
- Registered Psychiatric Nurse Grade 4 or Allied Health equivalent.
- Minimum of 3 years' experience in Acute Mental Health.
- Strong leadership, delegation, and prioritisation skills.
- Excellent communication and interpersonal skills.
- Sound knowledge of the Mental Health & Wellbeing Act 2022 and relevant legislation.
- Ability to work autonomously and within a multidisciplinary team.
- Current driver's licence.
- Previous experience as a Discharge Planning Nurse.
- Familiarity with NSQHS and National Standards for Mental Health Services.
