ITACS provides a time-limited, goal-oriented service for older people who would benefit from additional low intensity therapy in their own homes as well as assisting with the decision-making process around long term care service supports.
As the Physiotherapist in ITACS you will provide high quality Physiotherapy services to clients and carers, to ensure the provision of optimal health outcomes for the clients. You will be required to have problem-solving skills to support a client's safe mobility in the home and community setting without direct clinical supervision when completing in-home assessments and interventions. You will also provide care planning, case management and liaise with external stakeholders.
The position functions as part of a multi-disciplinary team with a teamwork approach to the provision of low intensity therapy home-based programs to frail elderly clients who are ready for discharge from sub-acute hospital care but are unable to return home without additional care and support.
For more information and to view the position classification, see the Position Descriptionhere.
