About the Role...

We are seeking a full-time (but open to part-time job share for the right candidates) Clinical Nurse Coordinator with an understanding of the NDIS Quality Standards. As a member of the clinical team, you will work closely with the Home Care Team to help manage aspects of clinical care within the business.

You will be overseeing a team of nurses and support workers and ensuring excellent high standard of patient care is received by all. You will assist in overseeing daily operations, developing and implementing policies and coordinate patient care with other healthcare professionals.

Responsible for the clinical assessment, provide ongoing clinical support advice (to informal and formal supports) and hands on clinical care as required. This includes close monitoring and support during times of health declines. You will be responsible for the development, maintenance and review of individualised support care plans for Home Care Clients, respite and short term accommodation.

You will need to ensure efficient and effective delivery of professional care and support from the Home Care Team to our Clients, respecting their rights in consultation and cooperation with the Client, their family/guardians and their key health professionals.

This role involves providing effective training, leadership, supervision and clinical support to the Home Care Team where necessary, ensuring care is provided in accordance with the relevant profession and industry standards, and all legislative requirements.

Main Tasks of the Clinical Nurse Coordinator...

Client Clinical Care

  • Conduct clinical assessments and provide care management
  • Create and monitor care plans and review them regularly.
  • Ensure high intensive support requirements (such as wound management, respiratory management, medication management, dysphagia etc) are met.
  • Client Clinical Care

Administrative Responsibilities

  • There is a high component of administrative responsibility. From completing and reviewing case notes from clients to monitor areas of concern/decline, undertake clinical assessments and provide reports to various stakeholders.
  • Incident management and mandatory reporting requirements.
  • Contribute to the creation, review and amend policies, procedures and other documentation when required.
  • Contribute to the creation, implementation and participate in promotional plans for the Home Care and Complex Care Programs.

Staff Supervision, Education and Training

  • Provide appropriate information, advice and support to staff with regards to clients' care plans.
  • Provide appropriate training and conduct competency assessments for internal and external Home Care employees in the specific cares of the clients.

Service Planning & Development

  • Actively contribute to service planning and improvement.
  • Initiate, develop and maintain ongoing contact and liaison with the primary health teams of our clients and other specified organisations.
  • Contribute to our growing services and clinical offerings.

Qualifications & Experience...

  • Current Nursing Registration with AHPRA
  • Relevant understanding and experience in acute health setting, delivery of health services within a community setting, complex case management, or palliative care
  • Certificate IV in Training and Assessment highly desirable.
  • Knowledge of and experience with the NDIS is highly desirable.
  • Ability to meet deadlines on a daily basis, manage competing priorities and tasks and delegate effectively.
  • Flexible approach to working hours (ability to cover leave and work care shifts to get an accurate understanding of clients needs in their own environment at various times of the day, attend expos and other events)
  • Previous experience in mentoring and supervising staff
  • Experience working with families within a health setting and commitment to the provision of family centred care preferred
