About the role: Lifestyle Facilitator (Support Worker)
Lifestyle Facilitators (Support Workers) are responsible for implementing support services aimed at maximising the living, employment, social and recreational skills of people with a disability. Community inclusion, independence, decision making, and personal choice should be promoted through these supports, with a focus on individual needs.

- Strong communication, interpersonal and advocacy skills including the ability to work and communicate effectively with managers, office staff and lifestyle facilitators, Participants and other stakeholders.
Experience:
- Demonstrated understanding of disability, mental illness, and the needs of carers of and Participants.
- Experience engaging with people who may have complex needs and behaviours.
- Strong coaching and mentoring skills
Required Certifications:
- Queensland Provisional Green or above Driver's License and comprehensively insured vehicle.
- Working with Children Check (Blue Card)
- Disability Worker Screening/NDIS Worker Screening (Yellow Card)
- Completion of NDIS Worker Orientation Module
- Nationally recognised First AID & CPR Certificate
- Two doses of the Covid vaccination
Desirable Knowledge and Skills:
- Experience working in Disability or Community Services sector.
- Experience working within supported independent living (SIL) residential environments.
- Ability to develop and maintain networks to a diverse range of service providers and client groups.
- Understanding of privacy and confidentiality issues.
Duties:
- Assist Participants to engage within the community, support personal development and endorse positive behaviour plans
- Take part in household tasks, meal prep, tidying, basic cleaning.
- Support participants who exhibit challenging behaviours and mental health conditions via understanding and implementing Positive Behaviour Support Plans
- Supporting participants with epilepsy and complex needs
- Undertaking personal cares; included but not limited to showering, toileting and dressing
- Manual handling: included but not limited to hoist transfers, slide sheets and wheelchairs
- Support Participants to reach their goals as outlined within their NDIS plan
- Adhere to relevant industry legislation and standards.
- Work within abbaCare's Policies and Procedures to ensure best practice
